Registration
Welcome to St. Mary's School. We are honored you chose our school to help educate your child. We would like to make sure this is an easy process for you. All forms, as well as everything you’ll need to register your child, are right here online. Registration is completed on-line using our student information system, Sycamore. Please call the office for more details at 402-291-1694.
Birth Certificates are required for all incoming preschool, kindergarten and transfer students from out of state. We must see the original birth certificate when you are enrolling your child. The official certificate should have a raised state seal. The secretary will make a copy of the original certificate and give the original back to you.
Health Exam - Kindergarten, 7th, and transfer students must have a physical. It is recommended all students get a dental exam and Kindergarteners and transfer students must have a vision exam.
Immunizations - All students must have a current and complete immunization record.
Baptismal Certificate for all students who have been baptized. Catholic students need this certificate to receive new sacraments.
Please inform the office throughout the year of any changes in contact information (cell phones, work #s, addresses, etc).
In addition to the above documents, ALL NEW students of St. Mary's School are asked to submit additional sacrament information. Parents of new students will also be asked to sign a release of information form so St. Mary's School can request records from the child's previous school. All transfer and new students will complete a registration form and submit it to the office. To register your child a deposit must be sent with your registration form. The deposit will be credited to your total registration bill. Registration Fees for students must be paid by July 1, 2016. That registration fee is used to pay for books, paper, and other materials needed to begin the school year.
The principal will review the application and an interview with the principal/pastor may be requested. The parent will be notified if the application cannot be accepted. Applicants who do not meet the admission requirements will be informed of the reasons for non-acceptance. An application that cannot be honored due to class size will be placed on a waiting list and parents will be notified of the status in writing. When the class size reaches 25 students a waiting list will be created.
Admissions- Non-Discrimination Policy - The Omaha Archdiocesan Schools admit students of any race, color, national and ethnic origins to all the rights, privileges, programs, and activities generally accorded or made available to students at the schools. They do not discriminate on the basis of race, color, national and ethnic origin and administration of their educational policies, admissions policies, scholarship and loan programs, and athletic and other school administered programs.
Once your child is enrolled at St. Mary's School you will receive a Family Handbook. Please sign the form on page 2. By signing the form, both parents and students commit to complying with the policies and guidelines of the handbook.
